Thank you for choosing Nova Eyecare. We value your time and make every attempt to see our patients in a timely fashion. You can help us in this by downloading and completing your Patient Registration Forms in advance of your visit. If you are unable to do so please arrive 15 minutes early to complete these forms in the office. Please be sure to bring your insurance cards and if you’ve been referred, your referral form. If you have a co-payment please bring it on the date of service.
Please bring your current eyeglasses, contact lens prescription if applicable, and any other documents that you think might assist us in your care. If you are transferring your care to our office, please bring as much of your medical record as you are able to obtain. If you are unable to acquire your records in advance, we will be happy to assist you with the necessary record release forms during your visit.
When scheduling your appointment please let the receptionist know the nature of your visit and how long it has been since you last saw us. This helps us schedule the proper amount of time and reduces the need for return visits.
If you need to cancel an appointment, please let us know as soon as possible. We appreciate at least 24 hours’ notice.
Nova Eyecare accepts Medicare as well as most major insurers for the state of Alaska. Most insurers offer a variety of plans which provide different benefits. We ask that our patients check their plan to be aware of their benefits.
Our doctors are both Vision and Medical Doctors who specialize in the diagnosis and treatment of eye diseases. Medical visits for will be billed under Medical insurance.
You may have a separate “Vision Insurance” in addition to Medical Insurance. “Vision Insurance” is generally provided for refraction and fitting of contact lenses and eyeglasses.
